Claim for Damages to Person or Property
Submit the completed claim form, below, and any attachments to the City Clerk's office at the following address, in person or by mail:

The original claim is then submitted to Human Resources / Risk Management for processing. Written responses are forwarded to the claimant from Human Resources / Risk Management within a 45-day period.
